2/6/17- Emailed her supervisor, Janet, and bumped date from 2/22/16 to 2/28/16.

3/8/17- Sent email to Janet for her to share with her staff. Asked that it be completed by 3/27.

4/6/17- Spoke with Janet to ask that it be completed by 4/15/17. She requested that I email the link to her (again!). Emailed and asked her to share it with all who are due.

4/13/17- Emailed final notice to Janet and will remove everyone at the end of the month.

5/8/17- Removed from the database.
